Liability Waiver & Media Release

By participating in classes, workshops, or events offered by Anastasia Tivari Dance Studio, you acknowledge and agree to the following:
Assumption of Risk
I understand that dance is a physical activity that involves inherent risks, including, but not limited to, muscle strains, sprains, falls, and other injuries.
I voluntarily choose to participate in classes and accept full responsibility for any risks associated with my participation.
Health Responsibility
I confirm that I am physically able to participate in dance activities. If I have any medical condition, injury, or health concern that could affect my participation, I understand that it is my responsibility to consult a physician before attending classes.
I agree to stop participating if I experience pain, dizziness, or any condition that may affect my safety.
Release of Liability
To the fullest extent permitted by applicable law, I release and hold harmless Anastasia Tivari Dance Studio, its owner, instructors, employees, contractors, and representatives from any claims, liabilities, damages, costs, or expenses arising from my participation in classes or use of the Studio's services, except where caused by gross negligence or willful misconduct.
Personal Property
The Studio is not responsible for the loss, theft, or damage of personal belongings brought to classes or studio locations.
Minors
Participants under the age of 18 may attend classes only with the consent of a parent or legal guardian. A parent or legal guardian must complete the booking and accept this waiver on behalf of the minor.
Photo & Media Release
During classes, workshops, or events, Anastasia Tivari Dance Studio may take photographs or videos for educational, promotional, and marketing purposes, including use on:

• The Studio's website

• Social media platforms

• Printed and digital promotional materials

By participating in classes, you grant the Studio permission to use photographs or videos in which you may appear without compensation.
If you do not wish to appear in photographs or videos, please notify the Studio in writing before your class, and we will make reasonable efforts to accommodate your request.
Agreement
By booking or participating in a class, I acknowledge that I have read, understood, and agree to this Liability Waiver & Media Release.
